The Home and Community-Based Services (HCBS) program is a New York State Department of Health (DOH) and Office of Mental Health (OMH) initiative dedicated to supporting children and youth aged 0-21 with medical fragility or serious emotional disturbances. HCBS aims to empower families by offering a comprehensive range of person-centered services designed to help youth thrive within their homes, schools, and communities, reducing the need for hospitalization or institutional care.

Yeled v’Yalda’s HCBS Department provides vital services for eligible children and youth enrolled in Medicaid. Our HCBS services help families address medical, emotional, and behavioral needs, promoting the successful integration of each child into their daily routines, school, and community. Through HCBS, we support caregivers, enhance children's independence, and foster their skills, preparing them for success in community living and beyond.

Our services are available in the five boroughs of New York City and throughout Nassau, Rockland, Suffolk, Sullivan, Ulster, and Westchester counties.

ELIGIBILITY

The HCBS program requires the following criteria for eligibility:
1. Children/youth aged 0-21;
2. Applicant must be actively enrolled in Medicaid;
3. Applicant must be a resident of New York State;
4. Applicant must have a qualifying medical or mental health condition.

SERVICES

Supporting caregivers in managing their child’s condition and building pathways toward independence. We assist with: * Individual needs to foster success at home or in the community; * Training in practical skills and strategies; * Promoting self-advocacy with other professionals; * Education on condition management and community resources.

Helping youth develop and enhance daily living skills. This includes activities that foster self-care, safety, communication, problem-solving, and social interaction.

Offering temporary relief to caregivers by taking care of the child’s needs, allowing families to recharge or manage other responsibilities.

Preparing youth for the workforce by teaching job readiness skills and introducing them to workplace expectations.

Supporting youth, aged 14 or older, in obtaining and maintaining paid employment, with a focus on skills needed for workplace success.
